Job summary

• Complete risk and needs assessments and review progress.
• Coordinate reintegration programs and support release planning.
• Arrange programs, referrals and support with prison staff and service providers.
• Maintain accurate records and program data for planning.
• Collaborate with Community Correctional Services for parole planning.
