Job summary

ForHealth’s Forest Hill Medical Centre is hiring a Registered or Enrolled Nurse to support chronic condition management and preventive care. You’ll work with GPs and a collaborative team to plan, review, and educate patients, focusing on CCM initiatives and patient outcomes.

Qualifications

• Current AHPRA registration as EN or RN.
• Prior experience in chronic condition management preferred.
• Strong communication and organisational skills.

Responsibilities

• Support and educate patients in managing chronic conditions and health behaviours.
• Conduct health assessments and promote preventative care.
• Collaborate with GPs and multidisciplinary teams, including referrals.
• Manage CCM appointment administrative tasks.
• Coach patients on sustainable health behaviour changes.
• Promote preventative healthcare initiatives (heart, women’s health, diabetes checks).
• Work with the team to grow the CCM program.

Practice Nurse - Chronic Conditions Management | Forest Hill | Part-time

About the Role

We’re seeking a Registered or Enrolled Nurse to join our Forest Hill Medical Centre, where you’ll support GPs and patients through chronic condition management (CCM) and preventative healthcare. You’ll be part of a great, supportive team that values collaboration, culture, and patient outcomes.

Key Responsibilities

• Support and educate patients in managing chronic conditions, and improving health behaviours through care planning and regular reviews
• Conduct health assessments and promote preventative care
• Collaborate with GPs and multidisciplinary teams, including referral coordination
• Manage administrative tasks related to CCM appointments
• Coach patients on sustainable health behaviour changes.
• Promote preventative healthcare initiatives across the practice, e.g. healthy heart checks, women’s health checks and diabetes checks.
• Work with the practice team to grow the CCM program.

About You

To be successful in this role, you will have:

• Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).
• Prior experience in Chronic Condition Management preferred
• Strong communication and organisational skills
