Job summary

ForHealth Group's Dapto Medical Centre is seeking a Registered or Enrolled Nurse to support GPs and patients in chronic condition management and preventative care. You'll join a welcoming clinical team focused on patient outcomes and a collaborative culture.

You will educate patients, conduct health assessments, coordinate referrals, and help grow the CCM program with a strong emphasis on prevention and quality care.

Qualifications

• Current AHPRA registration as EN or RN.
• Prior experience in chronic condition management preferred.
• Strong communication and organisational skills.

Responsibilities

• Support and educate patients in managing chronic conditions, and improving health behaviours through care planning and regular reviews.
• Conduct health assessments and promote preventative care.
• Collaborate with GPs and multidisciplinary teams, including referral coordination.
• Manage administrative tasks related to CCM appointments.
• Coach patients on sustainable health behaviour changes.
• Promote preventative healthcare initiatives across the practice, e.g. healthy heart checks, women's health checks and diabetes checks.
• Work with the practice team to grow the CCM program.
