Job summary

ForHealth Group in Adelaide is seeking a Registered or Enrolled Nurse to join Marion Domain Medical Centre. You will support GPs and patients through chronic condition management and preventative care, enhancing health outcomes in the local community.

Work with a supportive clinical team, participate in CCM program growth, and benefit from flexible part-time or full-time opportunities, with access to training and internal progression.

Qualifications

• Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).
• Prior experience in Chronic Condition Management preferred.
• Strong communication and organisational skills.

Responsibilities

• Support and educate patients in chronic condition management and regular reviews.
• Conduct health assessments and promote preventative care.
• Collaborate with GPs and multidisciplinary teams, including referral coordination.
• Manage administrative tasks related to CCM appointments.
• Coach patients on sustainable health behaviour changes.
• Promote preventative healthcare initiatives across the practice, e.g. heart checks, women’s health checks and diabetes checks.
• Work with the practice team to grow the CCM program.
