Job summary

ForHealth Group is seeking a Registered or Enrolled Nurse to join Springfield Medical Centre to support chronic condition management and preventative care. You will work with GPs and a multidisciplinary team to deliver patient-centered care and education.

The role offers part-time or full-time flexibility, a supportive clinical team, and opportunities for career progression through training and internal movement within a growing network of centres across Australia.

Qualifications

• Current AHPRA registration as EN or RN.
• Experience in Chronic Condition Management preferred.
• Strong communication and organisational skills.

Responsibilities

• Support and educate patients in managing chronic conditions and improving health behaviours through care planning and regular reviews.
• Conduct health assessments and promote preventative care.
• Collaborate with GPs and multidisciplinary teams, including referral coordination.
• Manage administrative tasks related to CCM appointments.
• Coach patients on sustainable health behaviour changes.
• Promote preventative healthcare initiatives across the practice, e.g. heart checks, women’s health checks and diabetes checks.
• Work with the practice team to grow the CCM program.

About the Role

We’re seeking a Registered or Enrolled Nurse to join our SpringfieldMedical Centre, where you’ll support GPs and patients through chronic condition management (CCM) and preventative healthcare. You’ll be part of a great, supportive team that values collaboration, culture, and patient outcomes.

Key Responsibilities

To be successful in this role, you will have:

• Strong communication and organisational skills
