Job summary

ForHealth Group is seeking a Practice Nurse at Forest Hill Medical Centre to support GPs with chronic condition management and preventative care. The role offers part-time hours with potential for full-time, depending on clinic needs, and a collaborative team focused on patient outcomes.

You will conduct health assessments, educate patients, and coordinate care with GPs and multidisciplinary teams, contributing to CCM reviews and quality improvements while enjoying staff development

Qualifications

• Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).
• Prior experience in Chronic Condition Management preferred
• Strong communication and organisational skills.

Responsibilities

• Support and educate patients in managing chronic conditions, and improving health behaviours through care planning and regular reviews
• Conduct health assessments and promote preventative care
• Collaborate with GPs and multidisciplinary teams, including referral coordination
• Manage administrative tasks related to CCM appointments
• Coach patients on sustainable health behaviour changes.
• Promote preventative healthcare initiatives across the practice, e.g. healthy heart checks, women’s health checks and diabetes checks.
• Work with the practice team to grow the CCM program.

About the Role

We’re seeking a Registered or Enrolled Nurse to join our Forest Hill Medical Centre, where you’ll support GPs and patients through chronic condition management (CCM) and preventative healthcare. You’ll be part of a great, supportive team that values collaboration, culture, and patient outcomes.
