Qualifications

• Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).
• Prior experience in Chronic Condition Management preferred.
• Strong communication and organisational skills.

Responsibilities

• Support and educate patients in managing chronic conditions, and improving health behaviours through care planning and regular reviews.
• Conduct health assessments and promote preventative care; collaborate with GPs and multidisciplinary teams, including referral coordination.
• Manage administrative tasks related to CCM appointments.
• Coach patients on sustainable health behaviour changes.
• Promote preventative healthcare initiatives across the practice, e.g. heart checks, women’s health checks and diabetes checks.
• Work with the practice team to grow the CCM program.

About the Role
