Job summary

ForHealth Group is seeking a Registered or Enrolled Nurse to join the Mount Druitt Medical Centre in Sydney. You will support GPs and patients through chronic condition management and preventative healthcare, working within a collaborative clinical team.

• Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).

Responsibilities

• Support and educate patients in managing chronic conditions, and improving health behaviours through care planning and regular reviews.
• Conduct health assessments and promote preventative care.
• Collaborate with GPs and multidisciplinary teams, including referral coordination.
• Manage administrative tasks related to CCM appointments.
• Coach patients on sustainable health behaviour changes.
• Promote preventative healthcare initiatives across the practice, e.g. healthy heart checks, women’s health checks and diabetes checks.
• Work with the practice team to grow the CCM program.

About the Role

We’re seeking a Registered or Enrolled Nurse to join our Mount Druitt Medical Centre, where you’ll support GPs and patients through chronic condition management (CCM) and preventative healthcare. You’ll be part of a great, supportive team that values collaboration, culture, and patient outcomes.

About You
