ForHealth Group in Mount Druitt, NSW, is seeking a Registered or Enrolled Nurse to support GPs and patients through chronic condition management and preventative care.

You will educate patients, conduct health assessments, coordinate referrals, and help grow the CCM program within a supportive clinical team. Flexible part-time or full-time roles and opportunities for training are available.

Qualifications

• Current AHPRA registration as Enrolled Nurse or Registered Nurse.
• Experience in Chronic Condition Management preferred.
• Strong communication and organisational abilities.

About the Role

We’re seeking a Registered or Enrolled Nurse to join our Mount Druitt Medical Centre, where you’ll support GPs and patients through chronic condition management (CCM) and preventative healthcare. You’ll be part of a great, supportive team that values collaboration, culture, and patient outcomes.

Key Responsibilities

• Support and educate patients in managing chronic conditions, and improving health behaviours through care planning and regular reviews
• Conduct health assessments and promote preventative care
• Collaborate with GPs and multidisciplinary teams, including referral coordination
• Manage administrative tasks related to CCM appointments
• Coach patients on sustainable health behaviour changes.
• Promote preventative healthcare initiatives across the practice, e.g. healthy heart checks, women’s health checks and diabetes checks.
• Work with the practice team to grow the CCM program.

About You

To be successful in this role, you will have:

• Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).
• Prior experience in Chronic Condition Management preferred
• Strong communication and organisational skills
