Job summary

Multicultural Care is seeking an experienced Physiotherapist to provide high-quality case management, support planning and care coordination across home care programs in a not-for-profit setting.

Based in Burwood, you will deliver patient-centered treatment plans, work with diverse clients, families and carers, and liaise with GPs and allied health professionals to ensure continuity of care and quality outcomes.

Qualifications

  • Bachelor of Physiotherapy or equivalent recognised in Australia.
  • Current full registration with AHPRA as a Physiotherapist.
  • Two years or more of clinical experience in relevant areas.
  • Interest in aged care and disability services and making a difference.

Responsibilities

  • Conduct comprehensive physiotherapy assessments and evaluations.
  • Develop and implement individualised treatment and rehabilitation plans.
  • Deliver evidence-based interventions to improve mobility and independence.
  • Monitor progress and adjust plans as required.
  • Provide rehabilitation following illness, injury, surgery, or discharge.
  • Coordinate care with case managers and multidisciplinary teams.
  • Maintain accurate clinical documentation and case notes.
  • Ensure compliance with standards and governance.

Job description

Multicultural Care is a leading not-for-profit organisation providing culturally sensitive home and community-based support to older people, people with disabilities, and their carers. We are seeking an experienced and values-driven Physiotherapist who is responsible for the provisioning of high-quality case management, support planning, and care coordination services across all home care program types (including SAH and CHSP).


This position will be working under Aged Care and Disability Service, who is responsible for providing Physiotherapy services to individuals, families and groups with a main focus on Consumer Directed Care services model for clients with diverse needs, including those living with disabilities. The position focuses on delivering personalised care plans tailored to individual needs, fostering independence, and improving the quality of life for clients who may be elderly, living with a disability, recovering from surgery or illness, or managing chronic conditions.

Key Responsibility Areas:

Clinical Services


  • Conduct comprehensive physiotherapy assessments and evaluations.

  • Develop and implement individualised treatment and rehabilitation plans.

  • Deliver evidence-based physiotherapy interventions to improve mobility, independence, and physical function.

  • Monitor client progress and modify treatment plans as required.

  • Provide rehabilitation and reablement services following illness, injury, surgery, or hospital discharge.

  • Support the management of chronic health conditions, musculoskeletal conditions, and age-related functional decline.

  • Maintain accurate and timely clinical documentation and case notes.


Client Care and Support


  • Educate clients, families, carers, and support staff on treatment plans and exercise programs.

  • Promote client-centred practices that respect individual choice, dignity, and independence.

  • Identify risks and implement strategies to minimise injury and enhance safety.

  • Support clients in achieving their therapeutic goals and outcomes.


Collaboration and Care Coordination


  • Work closely with Case Managers and multidisciplinary teams.

  • Liaise with General Practitioners, specialists and other allied health professionals.

  • Participate in case conferences and care planning discussions.


Compliance and Quality


  • Ensure compliance with professional standards, legislation, organisational policies, and clinical governance requirements.

  • Maintain registration and professional practice standards in accordance with the Physiotherapy Board of Australia and AHPRA requirements.

  • Participate in quality improvement initiatives, audits, and professional development activities.

  • Maintain client confidentiality and privacy at all times.


Skills, Experience and Competencies


  • Bachelor of Physiotherapy or equivalent qualification recognised in Australia.

  • Current full registration as a Physiotherapist with Australian Health Practitioners’ Regulation Authority (AHPRA).

  • Two years or more of clinical experience in relevant areas

  • A genuine interest for the people in aged care and disability services and are committed to making a difference to their lives.

  • Ability to work with a high level of independence and autonomy and accountability

  • Ability to effectively liaise with a diverse range of stakeholders and build strong relationship while maximising resource and collaborative opportunities

  • Ability to develop and maintain effective and empathic client centred therapeutic relationships with clients, families, carers and professional groups

  • Comprehensive understanding of new National Aged Care Quality and Safety Standard

  • Comprehensive understanding of the Support at Home Operational Manual and NDIS services

  • Experience in coordinating and allocating resources within budgetary requirements

  • Experience in preparing and participating in the periodic quality standards audit

  • High developed interpersonal skills with sound communication and literacy skills.

  • Ability to speak second language

  • Experience working in community/public health, disability under the NDIS or community-based setting
