HammondCare is seeking a Physiotherapist to join the Home Care Clinical Care & Practice Team in New South Wales. You will assess functional needs, set goals and deliver treatment to help older adults stay active and independent at home.

This role includes home visits or group activities for clients, working within a multidisciplinary team, and contributing to continuous quality improvement. Hybrid work options are available.

Qualifications

• You’re a tertiary qualified Physiotherapist with current AHPRA registration and practical experience.
• Experience in Aged Care, Inpatient, Community, Home-Based or Rehabilitation settings is valued.
• Strong communication and time-management skills; able to work independently in a multidisciplinary team.

Responsibilities

• Perform home visits or group activities for clients in Home Care or CHSP services.
• Assess functional care needs, set goals and provide treatment to maintain independence.
• Deliver person-centred care within HammondCare values and quality improvement framework.
• Travel between client homes and HammondCare locations with a valid driver’s license.

Job description

This is a permanent Full-time role and looks after clients living in the South Coast Region. You have the option to work from our Batehaven or Nowra office location.

As a Physiotherapist you'll be a valued member of our growing Home Care Clinical Care & Practice Team, working closely with an experienced, multidisciplinary team to provide tailored and individualised services to older adults living at home. A typical day will involve assessing functional care needs, setting care goals, and providing treatment and intervention, helping people stay active, independent and right where they belong - home.

You’ll be provided with a broad and varied caseload, with support from experienced clinicians. This meaningful role will include performing home visits or running group activities for clients accessing Support at Home or CHSP services, or residing in one of our Seniors Living units. You will deliver person-centred care in an environment that promotes continuous quality improvement, consistent with the values of HammondCare and the Australian Health Practitioners Regulatory Agency (AHPRA).

About you

• You’re a tertiary qualified Physiotherapist with current AHPRA registration and practical experience, or sound knowledge in Aged Care, Inpatient, Community, Home-Based or Rehabilitation or related setting.
• You’re a strong communicator, and able to plan and manage your time effectively.
• You enjoy working independently as part of a multidisciplinary team.
• You have access to a reliable vehicle, valid driver's license and safe driving record to travel between client’s homes and HammondCare locations (travel costs reimbursed).
