Job summary

UFS Healthcare is recruiting Pharmacists to deliver patient-centred care, clinical services and community health improvements in Ballarat, Victoria. This role focuses on clinical interventions, medication management and health education while collaborating with GPs and allied health teams.

You’ll use your clinical expertise to improve patient outcomes, participate in vaccination clinics and contribute to service development across the organisation.

Qualifications

• Qualified pharmacist with strong clinical knowledge and a passion for helping people.
• Excellent communication and patient counselling skills.
• Ability to work collaboratively within a team and take initiative.

Responsibilities

• Safe and accurate dispensing of prescriptions.
• Providing medicines information, counselling and health advice.
• Delivering professional services including vaccinations and MedsChecks.
• Identifying and recording clinical interventions to improve patient outcomes.
• Supporting medication management and preventative health initiatives.
• Maintaining high standards of professional practice and patient safety.
• Collaborating with healthcare professionals to provide coordinated care.
