Job summary

NSW Health at Hawkesbury District Hospital in Windsor, NSW, is seeking a registered Pharmacist to deliver clinical pharmacy services, promote medication safety and support medicines management across care transitions.

You will work with clinicians, community pharmacists and GPs to ensure continuity of care and optimal outcomes, providing therapeutic advice, medication reviews, and patient education while contributing to safety and quality initiatives.

Qualifications

• Tertiary qualifications in Pharmacy and current registration with the Australian Health Practitioner Regulation Authority (AHPRA).
• Post graduate clinical experience in a healthcare setting
• Excellent interpersonal, written, and oral communication skills.
• Ability to prioritise workloads, work independently and within an inter-professional team environment.
• Demonstrated high level of organisational and time management skills.
• Record of ongoing professional development.
• Computer literacy with experience in Microsoft Office (Word, PowerPoint, Excel, and/or Access) and Pharmacy Inventory and Dispensing programs.

Responsibilities

• Provide a clinical pharmacy service that promotes medication safety and supports best-practice medicines management across all transitions of care, from hospital admission through to discharge.
• Work closely with clinicians, community pharmacists and general practitioners to ensure continuity of care and optimal outcomes for patients.
• Provide expert therapeutic advice, undertake clinical medication reviews, and support patients and their families to use medicines safely and effectively.
• Contribute to medication safety and quality improvement initiatives, helping to enhance the delivery of high-quality, patient-centred care across the service.
