Job summary

Uniting AgeWell Geelong is seeking a Personal Care Worker to coordinate personal care for residents within care plans. You will work in a multidisciplinary team to provide clinical and emotional support, recognizing changes and escalating concerns to ensure wellbeing.

Roles include assisting with complex needs, supporting ongoing care planning, and maintaining high standards of resident safety and comfort. Night shift positions are available.

Qualifications

• Certificate III in Individual Support or equivalent (Aged Care) required.
• Experience in residential aged care/home care preferred.
• Experience with dementia and palliative care support desirable.
• Demonstrated ability to support residents in a fast-paced environment with strong communication.

Responsibilities

• Deliver high quality, person-centred care to residents and families.
• Provide personal care to residents with complex needs, including dementia and palliative care.
• Support nursing staff with assessment, care planning, documentation and evaluation.
• Monitor cleanliness, safety and environment; promptly report issues.
• Permanent Part Time Night Shifts available (Sunday to Wednesday, 23:00–07:15).

About the role:

At Uniting AgeWell, Personal Care Workers co-ordinate personal care for residents in accordance with care plans which meet their individual needs and preferences. Working within a multidisciplinary team, provide effective clinical and emotional support to residents, recognise changes in residents’ conditions and escalte observations and concerns.

• Deliver high quality, person centered care to residents and families, including consideration for cultural, spiritual and individual needs
• Provide personal care to residents with complex needs such palliative care, dementia support, restorative care and pain management to enhance their wellbeing
• Support nursing staff regarding assessment, care planning processes, documentation and evaluation of care outcomes
• Monitor the cleanliness, safety, environment and promptly report issues of concern
• Permanent Part Time Night Shifts available
  • Sunday to Wednesday, 23:00 to 07:15

About you:

• Certificate III in Individual Support, or equivalent (Aged Care)
• Experience in residential aged care/ home care
• Experience with dementia care and palliative care support (desired)
• Demonstrated abilities to support residents in a fast paced environment with exemplary communication skills in a team and one on one interactions with all levels of care partners
