Job summary

Uniting AgeWell Hawthorn is seeking passionate Personal Care Workers to join our dedicated team, delivering high‑quality, person‑centred care to residents in line with individual care plans. You will work in a multidisciplinary team, support nursing staff, manage complex needs including dementia and palliative care, and contribute to a supportive, values‑based workplace.

Casual and Part Time shifts available with flexible hours.

Qualifications

• Certificate III in Individual Support, or equivalent (Aged Care)
• Experience in residential aged care/home care (desired)
• Experience with dementia care and palliative care support (desired)
• Demonstrated abilities to support residents in a fast paced environment with exemplary communication skills in a team and one on one interactions with all levels of care partners

Responsibilities

• Deliver high quality, person centered care to residents and families, including consideration for cultural, spiritual and individual needs

About the role:

At Uniting AgeWell, Personal Care Workers co-ordinate personal care for residents in accordance with care plans which meet their individual needs and preferences. Working within a multidisciplinary team, provide effective clinical and emotional support to residents, recognise changes in residents’ conditions and escalate observations and concerns.

Our dedicated Hawthorn team is looking to add passionate personal care workers to their dedicated and professional care team.

We have currentCasualand Part Time opportunities available. Please see the vacant shift lines below.

• Wednesday to Saturday, 7am – 3pm
• Sunday to Wednesday, 7am – 3pm
• Wednesday to Saturday, 2:45pm – 10pm
• Sunday to Wednesday, 2:45pm – 10pm

In this role, you will:
