Job summary

Uniting AgeWell in Mornington is seeking passionate Personal Care Workers to join our Andrew Kerr Care Facility. This part-time role focuses on coordinating personal care, following individual care plans and delivering compassionate support to residents.

You will work within a multidisciplinary team, provide clinical and emotional support, and respond to changes in residents’ conditions. The position offers part-time shifts at the Mornington facility.

Qualifications

• Certificate III in Individual Support or equivalent (Aged Care).
• Experience in residential aged care/home care (desirable).
• Experience with dementia and palliative care support (desirable).
• Strong communication skills in team and one-on-one settings.
• Aligned with the BEST way of life (Believe, Excel, Support, Trust).

Responsibilities

• Deliver high quality, person-centered care to residents and families.
• Provide care for residents with complex needs including dementia and palliative care support.
• Support nursing staff with assessment, care planning and documentation.
• Monitor cleanliness, safety and report issues promptly.

About the role:

We are looking to add passionate and dedicated Personal Care Workers to our professional care team at our Andrew Kerr Care Facility.

We currently have the following permanent Part Time opportunities on offer:

07:00am to 15:00pm:

14:45pm to 22:00pm:

• Thursday, Friday and Saturday (both weeks)

At Uniting AgeWell, Personal Care Workers co-ordinate personal care for residents in accordance with care plans which meet their individual needs and preferences. Working within a multidisciplinary team, provide effective clinical and emotional support to residents, recognise changes in residents’ conditions and accelerate observations and concerns.

In this role, you will:

• Deliver high quality, person centered care to residents and families, including consideration for cultural, spiritual and individual needs
• Provide personal care to residents with complex needs such palliative care, dementia support, restorative care and pain management to enhance their wellbeing
• Support nursing staff regarding assessment, care planning processes, documentation and evaluation of care outcomes
• Monitor the cleanliness, safety, environment and promptly report issues of concern
• Part-Time opportunity working at our Andrew Kerr Care Facility in Mornington

About you:

This is the place for you if you have:

• Certificate III in Individual Support, or equivalent (Aged Care)
• Experience in residential aged care/ home care (desired)
• Experience with dementia care and palliative care support (desired)
• Demonstrated abilities to support residents in a fast paced environment with exemplary communication skills in a team and one on one interactions with all levels of care partners

Essentials for working in Aged Care:

• A current Working With Children Check card
• Current and ongoing flu and COVID-19 Vaccinations
• Ability to pass a national police clearance and NDIS Clearance
