Responsibilities

• Person-centered planning with client and family/advocates
• Complex physical care: mobility, transfers, positioning, daily tasks, devices
• Medical supports: monitor changes, follow plans, liaise with care team
• Social time and participation: provide meaning, fun, and community access
• Autonomy and advocacy: support decision-making, respect consent
• Cultural humility: respect backgrounds, beliefs, and ways of being
