Job summary

Claro Disability Services – Pillar Valley NSW seeks a compassionate Support Worker to assist clients in the Clarence Valley with mobility, daily tasks, and social participation. You will work closely with families, medical professionals, and other workers to deliver person-centered care that aligns with goals and routines.

Qualifications

• Candidate is person-centered, warm, patient and caring.
• Interest or experience in disability and health nuances.
• Great communicator who listens and adapts.

Responsibilities

• Chat with the client and family to understand goals and routines.
• Assist with mobility, transfers and daily tasks as outlined in the plan.
• Monitor medical plans, watch for changes and liaise with the care team.
• Create meaningful social time and community access aligned with interests.
• Support decision-making and promote autonomy while respecting consent.
• Respect cultural backgrounds and beliefs in everyday care.
