Job summary

Brain Hub is creating a part-time Patient Intake Coordinator to own the patient journey from first enquiry or referral through to commencement of care across adult and paediatric services. You’ll support families navigating neurological and developmental conditions with warmth and clarity, shaping a newly created function in our multidisciplinary clinics.

Qualifications

• Exceptional customer or patient experience skills.
• Experience in healthcare, allied health, disability or NDIS.
• Paid NSW Working with Children Check and National Police Check and NDIS Worker Screening clearance.

Responsibilities

• Managing new enquiries and referrals.
• Proactively following up prospective patients and families.
• Building rapport and understanding what people need.
• Explaining services, fees and next steps.
• Coordinating intake, onboarding and initial appointments.
• Supporting NDIS intake and administrative requirements.
• Maintaining an organised enquiry and referral pipeline.
• Ensuring a seamless handover into ongoing care.
• Collaborating with Care Coordinator, Patient Services and Clinicians to map the patient journey.
