Job summary

Mission Australia’s Country Recovery Connect service provides early intervention and support for children, young people, families and carers across the Riverland region who are impacted by alcohol and other drug use.

Based in Berri, this on-site role combines case management, outreach, advocacy and community engagement to deliver support in homes, schools and community settings where people feel safe and comfortable.

Qualifications

• Qualification in Community Services, AOD, Mental Health, Social Work or related field, or equivalent experience.
• Experience delivering case management services and supporting clients with complex needs.
• Understanding of alcohol and other drug issues and their impact on families.
• Ability to build strong relationships with children, families and stakeholders.
• Experience conducting assessments, developing case plans and managing risk.
• Knowledge of child protection responsibilities, mandatory reporting and safe practice principles.
• Excellent communication, advocacy, organisational and record-keeping skills.
• First Aid & CPR certificate and full driver licence.

Responsibilities

• Provide case management support to children, young people and families affected by alcohol and other drug use and other complex challenges.
• Conduct comprehensive assessments, risk assessments, case planning and regular case reviews to support positive outcomes.
• Respond to referrals from internal and external stakeholders and coordinate appropriate supports and service pathways.
• Deliver outreach support in community settings, homes, schools and other locations that best meet client needs.
• Advocate for clients and families by liaising with community services, government agencies and other key stakeholders.
• Facilitate community engagement activities, school education sessions and initiatives that promote awareness of available supports.
• Maintain accurate client records, case notes, reports and data entry in line with organisational requirements.

Education

Relevant qualification in Community Services, Alcohol and Other Drugs, Mental Health, Social Work or related field, or equivalent industry experience
