Job summary

Mission Australia’s Country Recovery Connect in Riverland, based in Berri, is hiring two part-time roles (3 days and 4 days per week) to provide case management and outreach support to families affected by alcohol and other drug use. You will work with children, young people and carers in Homes, schools and community settings.

Qualifications

• Relevant qualification in Community Services, Alcohol and Other Drugs, Mental Health, Social Work or a related field, or equivalent industry experience.
• Experience delivering case management services and supporting clients with complex needs.
• Demonstrated understanding of alcohol and other drug related issues and the impact they can have on individuals and families.
• Ability to build strong relationships with children, young people, families, community stakeholders and Aboriginal Community Controlled Organisations.
• Experience conducting assessments, developing case plans and managing risk within a client centred framework.
• Strong knowledge of child protection responsibilities, mandatory reporting requirements and safe practice principles.
• Excellent communication, advocacy, organisational and record keeping skills, with the ability to work effectively across rural and regional communities.
• First Aid & CPR certificate.
• Full driver licence with no restrictions.

Responsibilities

• Provide case management support to children, young people and families affected by alcohol and other drug use and other complex challenges.
• Conduct comprehensive assessments, risk assessments, case planning and regular case reviews to support positive outcomes.
• Respond to referrals from internal and external stakeholders and coordinate appropriate supports and service pathways.
• Deliver outreach support in community settings, homes, schools and other locations that best meet client needs.
• Advocate for clients and families by liaising with community services, government agencies and other key stakeholders.
• Facilitate community engagement activities, school education sessions and initiatives that promote awareness of available supports.
• Maintain accurate client records, case notes, reports and data entry in line with organisational requirements.
