Job summary

Apex Care is seeking an experienced Level 2 NDIS Support Coordinator to join our Melbourne-based team on a 3.5-day, hybrid schedule. You will manage an allocated caseload, help participants understand funding, and coordinate services to support independence and choice.

Qualifications

• Experience delivering NDIS Level 2 Support Coordination.
• Ability to independently manage a participant caseload.
• Strong communication and stakeholder engagement skills.
• Familiarity with NDIS plans, funding and guidelines.
• Proficiency with Microsoft Office and client management systems.

Responsibilities

• Manage allocated Level 2 Support Coordination caseload and develop/ review supports.
• Assist participants to understand NDIS funding, access services and exercise choice.
• Coordinate service providers, allied health professionals and stakeholders.
• Monitor participant budgets, funding utilisation and service agreements.
• Identify emerging risks and safeguarding concerns; adjust plans as needed.
• Prepare NDIS progress reports and maintain accurate case notes.
