Job summary

Wintringham is seeking an experienced Care Partner to join our Home Support team in Geelong. You will deliver person-centred care management and service coordination to help older people maintain independence and wellbeing in the community.

Responsibilities include developing care plans, coordinating services, managing budgets and records, and engaging with families, carers and providers to achieve positive outcomes.

Qualifications

• Experience in care management, case management or service coordination within aged care, health, disability, homelessness or community services.
• Ability to assess participant needs, develop care plans and coordinate services that support independence, wellbeing and quality of life.
• Strong administration, communication and stakeholder engagement skills, with experience managing budgets, records, compliance, feedback and complaints.

Responsibilities

• Support older people to live well by developing person-centred care plans that promote independence, wellbeing and quality of life.
• Connect participants with the right services and supports, ensuring care is responsive, coordinated and aligned with funding requirements.
• Build trusted relationships with participants, families, carers, service providers and community partners to achieve positive outcomes.
• Respond to changing needs by monitoring wellbeing, identifying risks and escalating complex matters when needed.
• Keep care on track by managing budgets, service agreements, records, compliance, feedback and complaints with care and accuracy.
