Job summary

Wintringham Limited is seeking flexible Casual Care Partners to backfill staff across our Home Support team in Victoria. You will travel across Geelong, Flemington, Braeside and other regions, delivering person-centred care and coordinating services to help older people maintain independence.

Responsibilities include developing care plans, connecting participants with services, managing budgets, and upholding funding requirements.

Qualifications

• Experience in care management, case management or service coordination in aged care or community services.
• Ability to assess participant needs and develop care plans to support independence and wellbeing.
• Strong understanding of person-centred practice and participant rights.

Responsibilities

• Develop person-centred care plans to promote independence and wellbeing.
• Coordinate services and supports across funding requirements and partners.
• Build trusted relationships with participants, families and service providers.
• Monitor wellbeing, identify risks and escalate complex matters as needed.
• Manage budgets, records, compliance and feedback to keep care on track.
