Job summary

Mounties Care is seeking a dedicated Care Partner to join our Dandenong office on a part-time basis (3 days per week). You will coordinate care plans, manage client budgets and assist clients to access My Aged Care, visiting clients in their home and community to ensure high-quality services.

The role requires 2–3 years in a similar position, valid First Aid and Police Check, and strong communication skills to liaise with clients, families and providers.

Qualifications

• Minimum 2–3 years’ experience in a similar role.
• Current clear National Police Check.
• Current First Aid Certificate, including CPR.
• Demonstrated knowledge within the Aged Care sector.
• Demonstrated care coordination skills and ability to work effectively with clients, family and service providers.
• Experience managing Home Care Packages.
• Experience navigating the My Aged Care Portal and understanding Aged Care Quality Standards.
• NDIS knowledge desirable; CRM and rostering system experience.
• Proficient computer skills, including Microsoft Office.
• Excellent verbal and written communication; time management and multitasking.

Responsibilities

• Develop and coordinate client care plans.
• Prepare individual client budgets.
• Assist clients to access My Aged Care.
• Visit clients in the community.
• Meet potential client leads and sign them up to our services.
• Participate in the after-hours on-call roster arrangement.
• Maintain consistency and continuity in the client care regime.
• Liaise with staff, clients, family members and healthcare providers as required.
• Maintain client confidentiality.
