Job summary

Mounties Care Home is seeking a dedicated Care Partner for a full-time role at our Hope Island, QLD office. You will coordinate care, help clients stay independent at home, and oversee a team of Carers to ensure quality services.

Key duties include developing care plans, budgeting, accessing My Aged Care, community visits, and engaging with potential client leads. You’ll also participate in after-hours on-call rosters and maintain confidentiality.

Qualifications

• Minimum 2–3 years’ experience in a similar role.
• Current clear National Police Check.
• Current First Aid Certificate, including CPR.
• Demonstrated knowledge within the Aged Care sector.
• Demonstrated care coordination skills and ability to work with clients, family and services.

Responsibilities

ABOUT THE ROLE

Mounties Care Home care is searching for a dedicated Care Partner to join our team on a full-time basis at our office in Hope Island, QLD.

The successful candidatewill work with our clients to assist them in remaining as independent as possible in their home and community. You will coordinate the delivery of care and services to our clients and have oversight over our team of Carers ensuring that quality services are delivered.

More specifically, some of your key duties will include:

• Develop and coordinate client care plans
• Prepare individual client budgets
• Assist clients to access My Aged Care
• Visit our clients in the community
• Meeting with potential client leads and ‘signing them up’ to our services
• Participate in the after hours on-call roster arrangement
• Maintaining consistency and continuity in the clients care regime
• Liaise with staff, clients, clients family and Healthcare providers where required
• Maintenance of client confidentiality

ABOUT YOU

The ideal candidate must hold the following as a minimum:

• Minimum 2-3 years’ experience in a similar role
• Current clear National Police Check
• Current First Aid Certificate, including CPR
• Demonstrated knowledge within the Aged Care sector
• Demonstrated care coordination skills and ability to work effectively with clients, family members and services providers
• Previous experience managing Home Care Packages
• Experience navigating the My Aged Care Portal
• Strong understanding of the Aged Care Quality Standards
• NDIS knowledge desirable
• Prior experience with CRM and electronic rostering system
• Proficient computer skills, including Microsoft Office
• Effective verbal and written communication skills
• Excellent time management skills, with the ability to multitask
• Positive and professional can-do attitude
