Job summary

Danila Dilba Health Service is seeking a Transient and Outreach Support Coordinator to join our multidisciplinary team in Darwin. This role focuses on assisting Aboriginal and Torres Strait Islander people experiencing homelessness with connections to mainstream and Indigenous services, coordinating outreach and referrals to improve health and social outcomes.

Applicants should have 3+ years in primary health care or equivalent, a Cert III/IV in a relevant field, and a willingness to obtain

Qualifications

• 3+ years working in primary health care or equivalent experience.
• Experience working with Aboriginal and Torres Strait Islander people experiencing homelessness and complex health issues.

Responsibilities

• Coordinate outreach and links to mainstream and Indigenous services.
• Facilitate referrals to services and coordinate client activities to achieve positive outcomes.
