The Salvation Army is seeking an Assertive Outreach Support Worker in Perth to provide mobile outreach and support services to people experiencing homelessness. You will assess needs, connect clients with housing and health services, and advocate within a trauma-informed framework.

This fixed-term role offers stability through to 30 June 2027 with potential extension. You’ll work in a dynamic team, maintain records, and participate in multi-agency collaborations to improve long-term housing

Formación

• Diploma in Community Services or related discipline is required or significant equivalent experience.
• Experience delivering welfare services to vulnerable people is essential.
• Knowledge of trauma-informed, strengths-based, and person-centred practice.

Responsabilidades

• Build authentic and respectful relationships with people experiencing homelessness through proactive outreach.
• Assess immediate and ongoing support needs and develop appropriate responses.
• Provide practical assistance including information, referrals, transport and material aid.
• Support individuals to access housing, health, mental health, alcohol and other drug, and community support services.
• Advocate for clients and work collaboratively with government, community and housing providers.
• Maintain accurate client records, case notes and service data.
• Undertake risk assessments and contribute to safe outreach practices.
• Participate in multi-agency initiatives that support coordinated responses to homelessness.
• Contribute to a positive, inclusive and high-performing team culture.
