Job summary

Plena Healthcare is seeking a qualified Occupational Therapist to join our Residential Aged Care team in Perth. You’ll provide personalised assessments and therapy interventions within a multidisciplinary setting, aligned with our Plena Care Pathway to tailor programs to resident goals and priorities.

• Bachelor’s degree is required.
• Current AHPRA registration is required.
• Australian working rights are required.
• Willing to obtain NDIS worker screening and National Police Check.

Responsibilities

• Provide comprehensive assessments and clinical recommendations to support consumer activities of daily living, mobility, transfers, falls risk and prevention, equipment requirements, pressure area care and skin integrity management, arthritic joint management, and pain management.
• Demonstrate flexibility and adaptability to the needs of Plena and the aged care facilities, and work within the scope of the Occupational Therapy profession.

Your Opportunity:

As a qualified Occupational Therapist working in Residential Aged Care, you will be joining a large and collaborative multidisciplinary team providing personalised treatments.

You will be involved in a new innovative way of offering allied health services to our wonderful Clients. Our Plena Care Pathway has been designed to provide our therapists with an opportunity to tailor therapy programs for our residents, built from resident goals and priorities, and delivers evidence-based mobility and functional programs, care plan reviews, one-to-one therapy interventions and group based active aging services.

Duties will include, but not limited to:

• Provide comprehensive assessments and clinical recommendation to support consumer activities of daily living, mobility, and transfers, falls risk and prevention, equipment requirements, pressure area care and skin integrity management, arthritic joint management, and pain management.
• Demonstrate flexibility and adaptability to the needs of Plena and the aged care facilities, and work within the scope of the Occupational Therapy profession.
