Job summary

Plena Healthcare is seeking a Physiotherapist to deliver mobility and functional therapy in Residential Aged Care, working with a multidisciplinary team across care settings. The role includes developing and reviewing care plans and delivering individual and group-based activities.

You will operate across RAC and community settings with mobility within a dedicated region. The successful candidate will hold General AHPRA registration, be willing to obtain checks, and have a current driver’s

Responsibilities

• Deliver mobility and functional therapy programs in Residential Aged Care.
• Conduct care plan reviews with the multidisciplinary team.
• Provide one-to-one interventions and group-based active ageing services.
• Collaborate with residents, families and facility staff as a key representative of Plena Healthcare.

This Physiotherapist role focuses on Residential Aged Care, working alongside a multidisciplinary team to deliver personalised, evidence-based therapy programs, including mobility and functional programs, care plan reviews, one-to-one interventions, and group-based active ageing services.
