Job summary

HammondCare is seeking an experienced Full-time Occupational Therapist to support aged care residents across dementia-specific residential homes. The role is based at Waratah (3 days), Cardiff (1 day) and Scone (1 day) with travel between sites; a high base pay rate or a fleet vehicle is available for travel to Scone, NSW.

Job description

HammondCare is currently looking for an experienced Full‑time Occupational Therapist to support Aged Care Residents living in our dementia‑specific residential homes across the Hunter.

This position will be based at Waratah (3 days per week), Cardiff (1 day per week) and Scone, NSW (1 day per week). Travel to Scone, NSW will be compensated with a high base pay rate, or access to a fleet vehicle located and parked at Waratah, NSW.

Overview

About the role – you will be part of a visionary, dynamic and supportive team seeking to make a significant difference in the lives of older people. You will develop, implement and deliver innovative individualised and group programs that promote healthy ageing, reablement and best‑practice dementia care. You will share your skills and experience with a great care team eager to learn, and work autonomously with residents to achieve the best care outcomes whilst collaborating with an interdisciplinary Allied Health team. You will deliver resident‑centred, evidence‑based care in an environment that reduces excess disability, promotes meaningful engagement and implements continuous quality improvement aligned with the values of HammondCare and the Australian Occupational Therapy Association.

Essence of the role
  • Provision of effective and timely occupational therapy to residents and appropriate support and advice to their representatives and the HammondCare team, at a service level and across residential care, to meet individual needs and preferences.
  • Collaborate closely with residents, their representatives, staff and the broader Allied Health Team to assess functional care needs, set care goals, and provide treatment and interventions to deliver the best possible outcomes for residents.
  • Participate as a member of the HammondCare team in the application of the Mission and values to pursue person‑centred, continuous quality improvement objectives.
  • Participate in quality improvement projects and continuing professional development.
Qualifications
  • Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action.
  • A tertiary qualification in Occupational Therapy and registration as an Occupational Therapist with AHPRA.
  • Practical experience or sound knowledge in Dementia (highly regarded) and Aged Care.
  • A current driver's licence, safe driving record and access to your own car, with a willingness to travel between HammondCare’s sites and services.
  • Effective communication and interpersonal skills, both written and verbal.
  • Demonstrated organisation and time‑management skills.
  • Demonstrated ability to work independently and as part of a multidisciplinary team.
