Queensland Government is seeking an experienced Nurse Practitioner to provide expert clinical assessment and specialist nursing care for people living with chronic disease across Central Queensland. You will lead advanced clinical practice, support evidence-based decision-making, and empower patients to better manage their health conditions.

The role requires NMBA Nurse Practitioner endorsement, 3+ years of advanced practice, and a commitment to education, mentoring, and research to advance

About us:The Sub-Acute Chronic Care Rehabilitation (SACCR) team provides interdisciplinary rehabilitative care to clients with a range of complex chronic diseases. These services also work to address unplanned medical admissions and support early discharge by enhancing the quality and availability of community based proactive collaborative care.
