Job summary

Central Queensland Hospital and Health Service in Rockhampton region is seeking an experienced Nurse Practitioner to provide expert assessment and specialist nursing care for people living with chronic disease across Central Queensland.

You will lead advanced clinical practice, support evidence-based decision-making, and collaborate with multidisciplinary teams to deliver safe, high-quality patient care. Qualifications include Master of Nursing and NMBA endorsement.

Qualifications

• Master of Nursing (Nurse Practitioner) with at least three years of advanced clinical practice in chronic disease.
• Current NMBA Nurse Practitioner endorsement with CQHHS scope and no restrictions.
• Commitment to education, evidence-based practice and mentoring.

About us

The Sub-Acute Chronic Care Rehabilitation (SACCR) team provides interdisciplinary rehabilitative care to clients with a range of complex chronic diseases. These services also work to address unplanned medical admissions and support early discharge by enhancing the quality and availability of community based proactive collaborative care. This service uses the interdisciplinary client centred model as described in the SACCR service profile, with student placement and clinical education embedded in this core business.

About the role

• Provide expert clinical assessment and specialist nursing care for people living with chronic disease across Central Queensland.
• Lead advanced clinical practice, supporting evidence-based decision-making, clinical governance and service development.
• Empower patients through education and support to better manage their chronic health conditions and improve outcomes.
• Collaborate across multidisciplinary teams to deliver safe, coordinated and high-quality patient care.
• Support the future of nursing practice through research, education, mentoring and development of Nurse Practitioner Candidates.

About you

• Master of Nursing (Nurse Practitioner) qualification with at least three years of advanced clinical practice experience in Chronic Disease; Specialist experience in Respiratory and/or Chronic Wound Care.
• Current NMBA Nurse Practitioner endorsement, meeting the Nurse Practitioner Standards for Practice (2021), with a CQHHS-endorsed scope of practice and no restrictions.
• Leadership and service improvement capability, using quality improvement, data analysis, problem-solving and innovation to strengthen services, drive best practice and respond to emerging healthcare needs.
• A strong commitment to education and evidence-based practice, supporting lifelong learning, mentoring others, undertaking research and using evidence to inform practice change.

What we offer

Total remuneration value of up to $188,624 p.a. is comprised of:

• An hourly rate between $79.85 - $83.37 p.h.
• 12.75% employer superannuation contribution
• 17.5% leave loading
