Central Queensland Hospital and Health Service - Rockhampton QLD is seeking an experienced Nurse Navigator (Generalist) to coordinate care for consumers with complex health needs. This advanced, autonomous nursing role will cut across hospital, community and primary care to reduce fragmentation of care.

You'll work with consumers, families and multidisciplinary teams, supporting health literacy, self-management and informed decision-making.

Qualifications

• Current AHPRA registration as a Registered Nurse is mandatory.
• Experience with chronic disease and/or acute or subacute nursing is preferred.
• Autonomous nursing role with leadership responsibilities.

The CQ Health Nurse and Midwife Navigator Service supports people living with chronic and complex health conditions across Central Queensland. Our Navigators work across hospital, community and primary care services to help people understand their care, connect with the right services and experience a more coordinated healthcare journey.

About the role:

As a Nurse Navigator (Generalist), you'll work with consumers who have complex healthcare needs, helping them navigate services and receive coordinated care across different settings and specialties.

This is an advanced, autonomous nursing role where you'll use your clinical expertise and knowledge of the health system to reduce barriers, avoid fragmented care and help consumers and their families make informed decisions about their health.

You'll:

• coordinate care across primary, secondary and tertiary health services
• work closely with consumers, families and multidisciplinary teams
• support health literacy, self-management and informed decision-making
• provide advanced clinical leadership and problem-solving
• contribute to quality improvement, service development, education and research.
• The role works across professional and organisational boundaries, so building strong relationships and bringing services together around the needs of the consumer will be a big part of your day.

About you:

You're an experienced Registered Nurse who is comfortable working autonomously, managing complexity and looking beyond one clinical area to see the bigger picture of a consumer's care.

You'll bring advanced clinical knowledge and experience in chronic disease and/or acute or subacute nursing, along with strong care coordination and multidisciplinary working skills.

You'll also have:

• strong clinical judgement, critical thinking and problem-solving skills
• the ability to communicate, negotiate and build relationships across services
• experience working across different specialties and healthcare settings
• leadership skills and a collaborative approach
• a genuine focus on improving the consumer experience and continuity of care.

Current AHPRA registration as a Registered Nurse is mandatory. A relevant postgraduate qualification, or working towards one, would be well regarded. A current Class C driver licence is also required.

What we offer:

Total remuneration value of up to $174,208 p.a. is comprised of:

• An hourly rate between $70.92 - $77.00 p.h.
• 12.75% employer superannuation contribution
• 17.5% leave loading
