Job summary

Royal Freemasons Ltd at Centennial Lodge Residential Aged Care seeks an experienced Clinical Care Coordinator (Grade 5) for permanent night duty, 9:30pm–7:30am, Tue–Thu. You will provide clinical leadership, coordinate resident care and mentor staff to deliver high-quality, person-centred care.

You will oversee assessments, planning and governance, working with a multidisciplinary team to ensure resident dignity and wellbeing during night shifts.

Qualifications

• Current AHPRA Registration as a Registered Nurse.
• Minimum of 5 years' experience as an RN in a clinical leadership role within aged care.
• Proficiency with clinical documentation systems.

Responsibilities

• Provide clinical leadership, support and mentorship to Team Leaders and care staff.
• Oversee resident assessments, care planning, care evaluations and clinical documentation.
• Ensure delivery of high-quality, person-centred care and best-practice clinical outcomes.
• Monitor and support residents with complex, acute, palliative and high-risk care needs.
• Oversee wound management, clinical audits and quality improvement initiatives.
• Assist with clinical reporting and compliance with aged care standards.
• Manage after-hours clinical issues, staffing requirements and roster backfilling as required.
