Qualifications

• Experience working with people with disability or in community services.
• Ability to follow NDIS plans and strategies.
• Ability to work autonomously and in teams.

Responsibilities

• Provide support to clients to achieve goals in their NDIS plans.
• Assist with community participation, skill building and daily living activities.
• Maintain accurate progress notes and documentation.
