Job summary

Pursue Ability Pty Ltd is seeking a part-time Support Coordination professional to grow and manage a local caseload under the NDIS framework in New South Wales.

You will generate referrals, coordinate supports, and ensure plans are delivered, with a target of 35 claimable hours per week when caseload is full.

The role emphasises timely communication, record-keeping, and compliance with NDIA requirements while building strong relationships with participants, families, and referral sources.

Qualifications

• Experience in NDIS Support Coordination or related roles.
• Ability to generate referrals and grow caseload.
• Current NSW driver licence and insured vehicle.

Responsibilities

• Generate and convert Support Coordination referrals into active participants.
• Build and hold relationships with referral sources (NDIA planners, plan managers, hospitals, clinicians, families).
• Maintain a live referral pipeline with source, status and outcomes in the client management system.
• Respond to new referrals within one business day.
• Identify participants with unspent funding and convert to active engagement where appropriate.
• Deliver Support Connection, Coordination of Supports and Specialist Support Coordination per plan and pricing rules.
• Achieve 35 billable hours per week at full caseload, measured over four weeks.
• Record service delivery with notes within 24 hours.
• Connect participants to supports, negotiate service agreements, and build capacity to direct their own supports.
• Prepare and lodge NDIA reports on time, with evidence and quality standard.
