Job summary

Y Whittlesea is seeking a part-time Support Coordinator to help individuals with disabilities navigate NDIS services in Horsham and surrounding areas. You will work with participants to implement their plans, connect them with providers, and support goal achievement.

Qualifications

• Certificate IV in Community Services or equivalent.
• Current Working with Children Check.
• NDIS Worker Screening Check.
• Current First Aid and CPR (or willing to obtain).

Responsibilities

• Coordinate services by implementing NDIS plans and connecting clients with providers.
• Set goals with clients and create action plans.
• Monitor progress and adjust plans to meet needs.
• Address issues or barriers to ensure client support.
