Job summary

Relative Care is seeking an NDIS Growth and Support Coordinator in NSW for a fulltime role. You will manage a cohort of support coordination clients while expanding the organisation’s services and maintaining strong relationships with referral partners and providers.

You will work as the vital link between participants, their families, support workers and the NDIS system, ensuring seamless service delivery and outcomes aligned with person-centred care.

Qualifications

• Experience in the NDIS sector or disability support services.
• Strong knowledge of the NDIS framework, participant planning processes and funding mechanisms.
• Excellent communication with participants, families, staff and external stakeholders.
• Ability to manage multiple participant cases and stay organized.

Responsibilities

• Develop and implement growth strategies to expand services across NSW.
• Grow participant enquiries from initial contact to service commencement.
• Maintain relationships with coordinators, hospitals, and referral partners.
• Coordinate service delivery and resolve issues across plans and providers.
• Monitor budgets, plan usage and provider performance.
• Support participants during reviews and advocate for their rights.
