We are a growing Registered NDIS Provider supporting participants across Sydney through Supported Independent Living (SIL), Community Access, Day Programs, Respite, Community Nursing and Allied Health services. Our focus has always been simple: providing genuinely person centred support while building meaningful relationships with every participant and family we work alongside.

As our organisation continues to grow, we are looking for an experienced Support Coordinator to join our close knit team.

This is not an entry level role. We are looking for someone who already understands the NDIS, enjoys building strong relationships and is confident managing a caseload independently from the outset. Support Coordinator (Full Time)

About the role

As a Support Coordinator, you will play an important part in helping participants understand and make the most of their NDIS plans. You will work closely with participants, their families and a wide range of professionals to ensure supports are coordinated effectively and participants are empowered to achieve their goals.

No two days are the same. One day you may be attending a plan review, the next you could be liaising with hospitals, therapists or the NDIA, advocating for a participant or helping to coordinate new supports. Throughout it all, your focus will always be on achieving the best possible outcomes for the people we support.

This is a full time office based position located in Caringbah.

Working hours are either 8.30 am to 4.30 pm or 9.00 am to 5.00 pm, Monday to Friday.

What you'll be doing
  • Coordinate supports for participants in line with their NDIS plans and individual goals.

  • Build genuine, long lasting relationships with participants and their families.

  • Confidently prepare participants for NDIS plan reviews and reassessments.

  • Gather supporting evidence and work collaboratively with participants and allied health professionals to maximise funding outcomes where appropriate.

  • Advocate for participants and ensure their needs and goals remain at the centre of every decision.

  • Build strong working relationships with allied health providers, hospitals, Local Area Coordinators, Plan Managers, the NDIA and other community organisations.

  • Work closely with our internal teams to ensure participants experience seamless and consistent support.

  • Maintain accurate case notes, documentation and records in line with NDIS requirements.

  • Identify challenges early and develop practical, person centred solutions.

  • Manage your caseload effectively while maintaining a high standard of communication and participant care.

About you

We are looking for someone who is confident, organised and genuinely enjoys working with people.

You will bring previous experience in Support Coordination and have a strong understanding of how the NDIS operates. You know how important communication is, you enjoy building professional relationships and you are passionate about helping participants navigate what can often be a complex system.

To be successful in this role you will have:

  • Previous experience working as an NDIS Support Coordinator.

  • A thorough understanding of the NDIS and current operational guidelines.

  • Experience preparing for and participating in NDIS plan reviews.

  • Excellent communication and relationship building skills.

  • Confidence working with participants who have a range of disabilities and support needs.

  • The ability to build strong relationships with allied health professionals and other external stakeholders.

  • Excellent organisational and time management skills.

  • High attention to detail and strong written documentation skills.

  • A positive, proactive attitude and the ability to think creatively when solving problems.

  • A genuine commitment to achieving great outcomes for participants.

Essential requirements
  • Current Australian Driver Licence.

  • NDIS Worker Screening Check.

  • Working with Children Check.

  • National Police Check.

  • Current First Aid and CPR Certificate, or willingness to obtain.

  • Full Australian working rights.
