Job summary

Care I Wish is seeking an NDIS Service Delivery Manager - SIL to lead a small team of Support Team Leaders, delivering care and attendant services to complex participants in SIL Homes. You will oversee end-to-end case management, conduct regular home visits and audits, and collaborate with external stakeholders to ensure service continuity and client satisfaction.

Qualifications

Experience in aged care/disability/community care.
Comprehensive knowledge of NDIS, disability sector, current reforms, funding and policy protocols.

Responsibilities

Support a small team of leaders to provide care and attendant services to complex participants in SIL Homes.
Manage end-to-end case management for each participant in the SIL portfolio.
Conduct regular SIL Home visits and audits to ensure safe systems of work.
Be available for external stakeholders, families and care teams to ensure continuity of service quality.
Coordinate documentation annually (or as required) to ensure compliant services across the SIL portfolio.
Identify areas of improvement and drive change initiatives.
Develop human resource capability to ensure safe delivery of NDIS client services.
Report within required timeframes on restrictive practices, chemical restraints and incidents.
