Job summary

One Door Mental Health is seeking an empowering Recovery Coach/Support Coordinator for a 1-year full-time contract delivering services across South West Sydney, including the South Coast and Southern Highlands.

You will support consumers to locate services within their NDIS plan, build independence, and navigate housing, health and education systems. The onboarding is KPI-free initially, with gradual uptake of billable hours and ongoing team support.

Qualifications

• Relevant qualifications in mental health or minimum of 2 years direct mental health experience.
• Ability to navigate and coordinate within the NDIS framework and community services.
• Strong stakeholder engagement and multidisciplinary collaboration.

Responsibilities

• Coach consumers with complex psychosocial diagnoses to build strengths, knowledge and decision-making capacity.
• Connect consumers to mainstream and community services including health and mental health outside the NDIS.
• Manage multiple, complex support structures with detailed support plans and identified risks.
• Assist consumers with budgeting funds and staying up to date on NDIS legislation and processes.
• Provide NDIA with reports on outcomes and success indicators as required.
• Transform lives by empowering individuals to achieve goals and improve quality of life.
