Job summary

Hope and Care Community Services in Brisbane’s northside is seeking compassionate Support Workers to assist individuals with intellectual and mental health needs, providing person-centred daily living support and community access. You’ll support participants in Morayfield, Deception Bay and Bracken Ridge with a focus on independence and wellbeing.

Flexible shifts including nights and sleepovers; ongoing training and development; and competitive hourly rates based on skills and experience.

Qualifications

• Experience in disability support, including mental health and psychosocial disabilities.
• Experience with daily living support and community access.
• Ability to follow individual support plans and administer medications as directed.
• NDIS Worker Screening Check is required before commencement.

Responsibilities

• Provide person-centred support with daily living, community access, and independence.
• Support participants with psychosocial, emotional, physical, and communication needs.
• Assist with medication, mobility, and transfers according to individual support plans and training.
• Maintain accurate progress notes, records, and incident reports.
• Follow prescribed mealtime management plans and Positive Behaviour Support strategies when required.
• Adhere to safe work procedures and the NDIS Code of Conduct.
