Job summary

Rising Up Support Pathways is seeking Disability Support Workers to assist participants in their own homes across Bundaberg, Hervey Bay and surrounding areas. You will contribute to person-centered care, helping clients live independently with dignity and choice.

Role requires sleepovers, weekend shifts and a willingness to work across day and night schedules. Training and development are provided, with a focus on safety, respect and autonomy for participants.

Qualifications

• Cert III in Individual Support or equivalent.
• NDIS Worker Orientation Module and NDIS Worker Basic Support Modules completed.
• Current NDIS Worker Screening Check and Blue Card.
• Driver's licence and reliable transport.
• Genuine availability for sleepovers, weekends and day shifts.
• Higher qualifications and experience with complex participants are highly regarded.

Responsibilities

• Provide personal care with dignity in home settings.
• Administer and document medications accurately.
• Support meal planning and preparation per dietary guidance.
• Assist with community access and scheduled activities.
• Write clear shift notes and progress reporting.
• Follow behaviour support plans when required and escalate safely.
• Maintain safety for participants and team across shifts.
