Job summary

Heart-led Support in Queensland seeks a Community Coordinator who connects participants, families and Disability Support Workers with the Operations Manager. You’ll ensure supports run in practice, not just on paper, and balance office tasks with community visits.

You’ll supervise DSWs, monitor rosters, chase details, and escalate risks early. A background in disability or community services, knowledge of NDIS guidelines, and a practical, no-nonsense approach are essential.

Qualifications

  • Demonstrated experience in disability, community services, participant coordination or frontline leadership.
  • Working knowledge of NDIS service delivery, the Practice Standards and the Code of Conduct.
  • Experience supporting frontline workers with guidance, feedback and follow-through.
  • Confidence with participant plans, risk information, routines and stakeholder communication.
  • Clear, factual writing. Notes, records and management updates that hold up.
  • Current NDIS Worker Screening Check, driver licence and reliable vehicle.
  • Cert IV or Diploma in Community Services, Disability or Individual Support preferred.

The Role: Community Coordinator

You're the connection point between our participants, their families, our Disability Support Workers and the Operations Manager.

You make sure supports are working in practice, not just on paper.

Some days that's phone calls, records, rosters and follow-up from the office. Other days it's a participant's lounge room or a community outing. You hold the relationships, you back our DSWs, you chase the details, and you flag things early.

What You'll Be Doing:

Regular check-ins with participants, families and stakeholders, in person and in the community

Day-to-day supervision, coaching and practical feedback for our DSWs

Watching service coverage and working roster gaps before continuity is affected

Matching the right worker to the right participant

Checking documentation, notes and care plan records actually stack up

Following up quality gaps, low-level complaints and routine service issues

Spotting risk, safeguarding and pattern concerns and escalating them early

Keeping the Operations Manager across what's really happening on the ground

You Must Have;

Demonstrated experience in disability, community services, participant coordination or frontline leadership

Working knowledge of NDIS service delivery, the Practice Standards and the Code of Conduct

Experience supporting frontline workers with guidance, feedback and follow-through

Confidence with participant plans, risk information, routines and stakeholder communication

Clear, factual writing. Notes, records and management updates that hold up

Current NDIS Worker Screening Check, driver licence and reliable vehicle

Cert IV or Diploma in Community Services, Disability or Individual Support preferred. Real experience counts
