Aboriginal & Torres Strait Islander Community Health Service Brisbane is seeking an entry-level NDIS Care Coordinator in Brisbane to learn under experienced coordinators. You will support participants in understanding NDIS plans, connect them with services, and help draft simple action plans."

The role offers full training, mentorship, and a path toward a future as a Support Coordinator within our culturally safe, community-focused team.

Qualifications

• Certificate IV or higher qualifications in Health Care, Social Assistance, Community Services, Administration and/or one year disability/age care experience.
• Demonstrated ability to record accurate and consistent case notes.
• Ability to work with a multi-disciplinary team and contribute to developing services.
• Strong organisational skills and ability to work independently or in a team.
• Ability to communicate with sensitivity with Aboriginal and Torres Strait Islander people.
• Current Drivers Licence (QLD).
• Current National Police Clearance or ability to obtain.
• Working with Children’s Card (Blue Card) or be eligible to apply.
• Disability Worker Screening (Yellow Card) or be eligible to apply.
• Willing to travel to our other locations.

Responsibilities

• Support participants in understanding NDIS plans and available supports.
• Assist linking participants with service providers, resources, and culturally safe supports.
• Develop simple action plans with a focus on short-term goals.
• Provide administrative support, including case notes and policy compliance.
• Attend meetings with participants, families, and stakeholders to gain experience.
• Develop culturally safe, person-centred approaches to service delivery.
• Participate in training and mentorship to build future career progression.
