Australian Workforce seeks a dedicated Community Support Worker to help adults living with disability start their day safely and confidently in Concord, NSW. You’ll work morning shifts finishing by 10:00 AM and participate in a weekly sleepover shift.

Join us to support residents in independence and community participation with person-centred care, while adhering to NDIS guidelines. A driver’s licence, First Aid, and a compassionate, team-oriented mindset are essential.

Qualifications

• Experience in community support or disability services.
• Certificate III or IV in Individual Support/Disability.
• Current First Aid certificate and a valid Australian driver’s licence.
• Confident with medication administration and supporting restrictive practices.

Responsabilités

• Provide personalised care based on individual goals and care plans.
• Assist with personal care, meal preparation, and essential household tasks.
• Facilitate social participation through recreational and leisure activities.
• Ensure clients’ rights, safety, and wellbeing are protected at all times.
• Maintain accurate records in line with NDIS and organisational policies.
• Collaborate with team members and external stakeholders to deliver coordinated care.
