Job summary

Sibella Care in Sydney is seeking a compassionate Mental Health Support Worker to assist participants with complex mental health needs and disabilities in community settings. This role includes risk assessments, activity planning, and support aligned with individual NDIS plans.

You will work across Western Sydney to North Shore areas on a casual basis, with a strong emphasis on person-centred care, documentation, and ongoing professional development.

Qualifications

• Minimum Certificate III in Individual Support or equivalent.
• Certificate IV in Mental Health preferred.
• Experience in mental health or disability support is desirable.

Position Overview

The Mental Health Support Worker plays a crucial role in supporting participants with complex mental health needs and disabilities. This role includes conducting risk assessments, working within open community environments, supporting participants in achieving their goals, and ensuring activities are engaging, safe, and aligned with individual support plans.

Key Responsibilities

• Design and implement activities that align with NDIS participant needs and goals.
• Conduct thorough risk assessments for all activities to ensure participant safety.
• Collaborate with the Head Coordinator for Community & Centre-Based Day Programs to plan and schedule activities.
• Provide activity support to ensure participants are engaged and participating to their fullest potential.
• Assist participants in working towards personal goals as outlined in their NDIS plans.
• Maintain accurate and up-to-date records of participant progress, incidents, and daily logs.
• Communicate and collaborate effectively with participants, their families, and other stakeholders.
• Complete all required documentation, including daily logs, communication books, and incident forms, accurately and on time.
• Engage in ongoing professional development, including training and relevant courses.
• Conduct personal care duties safely, with a focus on participant-centred care.
• Work with participants with psycho-social and mental health challenges, including those displaying complex behaviours.
• Assist the organisation in gaining new referrals.
• Approach work with an open values framework, respecting participant values even when they differ from your own.
• Perform additional duties in line with the role as required.

Qualifications And Experience

• Minimum Certificate III in Individual Support or relevant qualifications in Disability Support, Community Services, or related fields. Certificate IV in Mental Health is preferred.
• Prior experience as a support worker in mental health-focused environments, community outreach, or residential settings.
• Strong skills in conducting and updating risk assessments.
• Excellent written and verbal communication skills, with the ability to liaise professionally with diverse stakeholders.
• In-depth knowledge of the NDIS, including operational guidelines and effective utilisation of NDIS packages for participants.
• Current driver’s license, access to a car, NDIS worker screening clearance, NSW Working with Children Check, and First Aid and CPR certification.
