Job summary

Conscious Care Group, a boutique NDIS provider delivering SIL and CAS, is seeking experienced Support Workers in Queensland. You will support participants living with complex mental health needs such as ASD, BPD, DID, PTSD, and Schizophrenia, following behaviour support plans and safety protocols.

You will complete shift notes and incident reports, assist with daily living and community access, and work closely with the team to ensure high-quality care and safe outcomes.

Qualifications

• Experience supporting individuals with complex behaviours in mental health.
• Clear spoken and written English, including accurate shift notes and incident documentation.
• Solid understanding of NDIS requirements and Australian workplace expectations; follow behaviour support plans and risk management.
• NDIS Worker Screening Check, Blue Card, First Aid/CPR; valid driver’s license and access to a vehicle.

Conscious Care Group is growing, and we're looking for experienced Support Workers who are ready to take the next step in their career.

We are a boutique NDIS provider delivering Supported Independent Living (SIL) and Community Access Support (CAS) to participants with a range of mental health needs.

About the Role

You will support participants living with complex mental health conditions, which may include ASD, BPD, DID, PTSD and Schizophrenia. Some participants experience complex or challenging behaviours. Training, guidance, and clear support plans are provided.

Your role will include:

• Providing person-centred support in line with care and behaviour support plans
• Supporting participants with daily living and community access
• Completing clear and accurate shift notes and incident documentation
• Working collaboratively with your team and leadership for safe outcomes

Shifts Available (not limited to)

• Day shifts: 6:00am - 3:00pm
• Evening shifts: 3:00pm - 10:00pm

What You'll Bring

To be successful in this role, you must demonstrate:

• Experience supporting individuals with complex behaviours in mental health support
• Clear spoken and written English, including the ability to complete accurate shift notes, incident reports, and behaviour documentation to a high standard
• Strong understanding of NDIS requirements, professional boundaries, and Australian workplace expectations
• The ability to follow behaviour support plans, risk management strategies, and restrictive practice protocols precisely
• NDIS Worker Screening Check (Yellow Card)
• Working with Children Check (Blue Card)
• Current First Aid and CPR (or willingness to obtain)
• Driver's licence and safe, insured vehicle
• Legal working rights in Australia
