HammondCare Cardiff is seeking an experienced Permanent Registered Nurse to join our team in a dementia-specific residential care home.

In a part-time clinical role, you will mentor and support a team of specialised dementia carers, contribute to care planning, and provide clinical assessments within our Model of Care.

We value patient-centred, relationship-based care and offer ongoing training and development through industry-leading programs.

Qualifications

• Current registration with AHPRA as a Registered Nurse.
• Ideally 2–3 years' experience within an Aged Care environment.
• Passion for improving the quality of life of older people living with dementia.
• Strong communication and interpersonal skills.
• Ability to work collaboratively within a multidisciplinary team and show initiative.
• Patience, empathy and a commitment to person-centred care.

Responsabilités

• Mentor, lead and support a team of specialised dementia carers.
• Partner with clinical leaders to ensure high-quality resident care.
• Provide clinical assessments and contribute to care planning.
• Support clinical decision-making and nursing oversight.
• Implement and promote comfort-focused care approaches.
• Build relationships with residents in line with HammondCare's Model of Care.
• Champion best practice dementia care and support continuous improvement.
